Model 1: Compounded Medication Included
The most common model. One monthly price covers medication, provider review and usually shipping. Watch for whether the price rises as you titrate.

The Titration Question
Some of these hold one price across every dose, notably Precision Telemed and NexLife . Others climb steeply: VITAstir runs from $145 at 2.5 mg to $599 at 15 mg. Aspire Health publishes the full ladder, from $149 a vial at 2.5 mg to $299 at 15 mg. Our flat-rate pricing comparison works through which providers hold the price. A provider with a higher entry price and a flat structure can easily cost less over a year than one advertising a low starting rate.
Model 1b: Retail and Warehouse Pharmacy
A newer route that now competes directly on price. Sam's Club Pharmacy and Kroger Pharmacy both publish eligible GLP-1s as low as $25 a month with insurance and around $149 self-pay for the oral formulation, with a $50 route for eligible Medicare members. That buys an FDA-approved product rather than a compounded one, and it undercuts a large share of the compounded market. The trade-off is that these are dispensing pharmacies: you need a prescriber elsewhere, and no titration support is included. Our retail pharmacy comparison sets out each chain.
Model 2: Consultation Only, Medication Separate
You pay for clinical time; medication is billed by a pharmacy. Total cost depends entirely on where and how you fill, which makes this the best model if you have insurance or intend to use a manufacturer channel.

The spread here is enormous for identical medication. Manufacturer channels are dramatically cheaper than resale pricing.
Model 4: Membership Plus Medication
A few providers charge a platform or membership fee separately from medication, so the advertised medication price is not your monthly cost. Shapely publishes $99 a month membership, discounted to $49 on sale, on top of $99 semaglutide, meaning a realistic monthly figure closer to $148 to $198. Always ask whether membership is included or additional.
What Is Not in These Numbers
- Lab costs, unless the provider includes them. Shapely publishes $75 at Labcorp; Fridays includes Quest labs at no charge
- Supplies such as needles and swabs, unless stated as included
- Shipping, though most providers now include it
- The cost of prepaying a year and stopping early, which is generally not recoverable
- Promotional first-month rates that revert to a higher standing price
The Cheapest Route by Situation
Eligible Medicare Part D member
The Bridge Program at $50 a month is the cheapest legitimate route to FDA-approved medication we have found, through 31 December 2027.
Commercially insured
Manufacturer savings cards can reach $25 a month subject to caps. Pursue coverage through Klinic or Midi Health .
Self-pay, wants FDA-approved
The NovoCare Wegovy pill at $149 a month is the lowest published approved-product price.
Self-pay, compounded is acceptable
Compounded semaglutide starts around $99 to $119 a month on longer commitments. Compare at maintenance dose, not entry dose.
Verify Before You Buy
During research for these reviews we found several comparison sites quoting Sprout Health at $249 and $299, when Sprout's own site publishes $149 and $199. Pricing in this market changes often and aggregators go stale. Confirm with the provider directly before committing, and treat every figure here, ours included, as a starting point rather than a quote.

What is the catch with annual prepay pricing?
The lowest advertised rates usually require paying 12 months upfront. NexLife's $119 rate requires $1,428 in advance. If you stop at month four, that money is generally not recoverable, so confirm the refund position first.
